2/22/08 Update de 22/2/08

We have no images to post today, as Antonio had a brachycardia episode when we were there to visit him.

It was time for his cares, and I was there at his bed side getting ready to change his diaper, and Melissa changed his position very gently, but as soon as she put him down, his heart rate started to drop quickly. from 170 to 120 to 95 to 75 ... that was when I moved away from the bed and started praying, as Melissa (his nurse) scrambled to call for help.

It was very scary and extremely upsetting, especially since Melissa seemed to be particularly concerned, with a deer-in-the-headlights face throughout the episode... But in the end, they were able to make him stable again, we did his cares and he went back to sleep without setting the alarms off again all day long!

Here are some numbers for you: a few days ago they uped the number of calories added to his milk, and now he's getting 26 cal/oz of milk, and that is really paying off -- he is weighing now 2 Lbs 11 OZ!!!

So, the rest of the post will be a little technical, to report on a meeting we had yesterday with various people involved in getting Antonio better... It is what they call a head-to-toe-to-bank, when they discuss all aspects of his case, including the financial issues (Social Security and Medical Assistance). So here it goes:

  1. Starting from his head, he had the third brain sonogram and all looked good, and as far as the doctors can see, there are no signs of any hemorrhage or damage that can cause PVL. They would not go as far as saying he will have normal development, but they said the next best thing: at this stage there is no reason to believe that he will have definite problems.

  2. Eyes: They did his first eye exam yesterday (after the meeting), his retinas look good and they did not see any signs of ROP so far. They will repeat the exam in two weeks just in case as he still can get ROP.

  3. Lungs: As you can guess from all previous posts, that is the major issue we had so far. He definitely has BPD and RDS, which is normal for premature babies. His lungs look much better and he's now able to breathe on his own, but the concern is that he is "stuck" with some support of the ventilator, which is not good on the long term.

    Since he got off of the oscillator, they have seen a lot of thick, cloudy secretions coming out from his ET tube, requiring them suction the tube constantly during the day. The secretions were the reason he failed on the CPAP and it is not clear if that secretions are from an infection or inflammation.

    So they are treating him with antibiotics, in addition to several inhaled medications (pulmicort, albuterol, tobramycin, the last one also an antibiotic) to heal his lungs.

    The next step, in case he does not improve in the next week or so, will be to start him on steroids, most likely hydrocortisone for 1-3 weeks. The problem with the treatment course is that studies show that, although steroids were thought to be miraculous drugs for a long time, they are now used more cautiously as recurrent courses and high doses have revealed serious side effects, including impaired development and cerebral palsy. Still, in situations of survival, steroids can provide amazing results.


  4. Belly: As mentioned above, he is taking his feedings very well, even with the increased caloric intake, in addition to the human milk fortifier, Antonio is also getting MCT oil, Sodium and Potassium with his daily feeding. There is still a risk for NEC, but studies show that Babies who are fed breast milk have a lower risk of developing NEC.

  5. Infections: Since premature babies have immature immune systems, they are more susceptible to bacteria, viruses and other organisms that can cause infection. The doctors are specially worried about RSV and for that he gets a once a month shot of Synergist, a $1,200-a-pop injection that supposedly warns off the RSV virus.

Nós não temos nenhuma foto pra postar hoje, porque o Antonio teve um epsódio violento de braquicardia ontem quando eu estáva o visitando. Era quase a hora de seus “cuidados” e estava ao lado da cama dele, me preparando pra trocar a fralda. A enfermeira Melissa delicadamente trocou a posição dele, mas assim que o colocou de volta no colchão, o coraçãozimho dele começo a bater mais devagar, e a frequência cardíaca caindo rapidamente de 170 à 120 à 95 à 75...

Nesse momento, eu saí de perto da cama e comecei a rezar, enquanto a Melissa tentava manter a respiração do Antonio e chamava por socorro dos médicos e outras enfermeiras de plantào.

Par mim isso tudo foi bem assustador (foi bom que a Sandra estava fora da sala bombeando), principalmente por que a Milissa tinha um olhar desesperado o tempo todo uqe durou o episódio... Mas no fim, conseguram estabilizar o Antonio, nós cuidamos dele (fralda, temperatura, sucção, etc...) e ele voltou a dormir mais uyma vez, sendo que não tocou mais nenhum dos alarmes outra vez pelo resto do dia!

Seguem algums números para você: Há alguns dias, os médicos almentaram o número dos calorias que estão sendo adicionados ao leite, e agora oi Antonio está tomando aproximadamente 0.88 cal/mL de leite e, com isso, ele ganhou mais peso e está agora com 1.210 g!

Assim, o resto do post será um pouco técnico, um relatório de uma reunião que nós tivemos ontem com as vários pessoas envolvidos no caso do Antonio ... É o que eles chamam de reunião “do cucuruto à ponta do pé, passando pelo banco”-- discutimos todos os aspectos do caso em detalhe, incluindo as finanças (segurança social e auxílio médico):

Começando pela cabeça, fizeram o terceiro ultrasom do cérebro e tudoparacia bem e os médicos não viram nenhum sinal de hemorragia ou danos hage ou danos que puder causar PVL. Como era de ser esperam, não nos garantiram que ele terá um desenvolvimento normal, mas disseram a próxima melhor coisa: neste estágio não existe nada que os façam acreditar que terá problemas definitivos.

Olhos: Finalmente fizeram seu primeiros exame ocular ontem (após a reunião), e tudo parace normal com a retina e não viram nenhum sinal de ROP. Vão repetir o exame daqui há duas semanas, uma vez que ainda existe chance de desenvelver essa doença

Pulmões: Como você pode deduzir pelos últimos posts, os pulmões são nossa maior preocupação nesse momento. O Antonio tem BDP e SRD o que é normal com bebês prematuros. Seus pulmões paracem muito melhor e ele vem respirando pro conta própria há algum tempo. No entanto, ele parace ter “ficado viciado” no suporte do ventilador, o que não é bom ao longo prazo.

Desde que saiu do VOAF, os médico viram um aumento de secreções (leitosa e grossa) vindo do tubo do ventilador, requerendo a sucção do tubo constantemente durante o dia. Essa secreção foi a razão porque o Antonio “falhou” no CPAP, e os médicos não sabem se ela é decorrente de uma infeccção ou inflamação apenas.

Assim que o estão tratando com os antibióticos, além dos diversas inalações (pulmicort, albuterol, tobramycin, este último também um antibiótico) para curar os pulmões. A próxima etapa, caso ele Nõ apresente melhoras até a semana que vem será a utilização de esteroides, provavelmente com a hydrocortisona por 1 até 3 semanas. O problema com esse tratamento é que alguns estudos recentes mostram que, embora os esteroides tenham sido descritos com medicamentos milagorsos até há não muito tempo, agora são usados com mais cautela uma vez que o uso continuado e dosagem elevadas revelaram efeitos colaterais sérios, incluindo a paralisia cerebra. Mesmo assim, em situações com a do Antonio, os esteroides podem trazer resultados surpreendentes.

Estomago:: Como mencionado acima, o Antonio está indo muito bem no quesito alimentação, mesmo com o aumento das calorias no seu leite. Além do forticante do leite materno, ele ainda recebem também o óleo de MCT, sódium e o potassio com a sua alimentação diária. Ainda existe o risco dele contrair NEC, mas estudos mostram que os bebês que são alimentados com leite materno têm um risco mais baixo de contrair essa doença.

Infecções: Uma vez que os bebês prematuros têm o sistema imunológico imaturo, ele são mais suscetíveis às bactérias, vírus e aos outros microorganismos que podem causar infecções. Os médicos são preocupados especialmente com a RSV e para evitá-la estão dando uma vacina mensal (Synergist) que custa US$1,200 cada aplicação...

....Dá para imaginar a situação que você viveu e nehum de nós por perto.....chega a dar vontade de ir até aí a jato,e voce sabe que se pudesse já estaria aí com certeza.muita força nessa hora pois voc^}e é o pilar de tudo,como sempre foi e com certeza o Antônio já percebeu isso e se sentir o Pai para baixo ele tb ficará,assim como a Sandra. nesses momentos feche os olhos e imagine todos seus amigos ao seu lado rezando e torcendo para esses momentos de angústia se abreviarem logo.
