New Update

Sorry, we've been quiet for the last few days, but I was out of town, and it was a little difficult for me to post any updates.
I will start by adding some new pics we took last Sunday, in which Antonio is sporting the same nice bootie-hat combo -- the boy is already a fashionista like mom and dad! These shots were taken after dad changed his first wet diaper... on the detail, Nurse Gail checking Antonio's heart beat.

Since then, there were many changes, not all of them for the best.... remember the roller-coaster analogy

Since Antonio gained more weight -- he's now 930 grams or 2.053 Lbs -- they have increased the volume of milk he is given to 5.3 mL/hour (a little more than one teaspoon!). That constitutes a full feeding for a baby with his size, and in addition they are also giving him MCT oil to add some calories and help him gain more weight.

On Tuesday night he was put back on the conventional ventilator, which would be definitive a step towards graduating him to breathing on his own. However, after 48 good hours on the conventional ventilator, Antonio was requiring more BPM (breaths-per-minute) and higher concentration of 02, meaning that blood gases were "out-of-whack" and that the level of CO2 was rising... so Dr. Bennett decided to put him back on the High Frequency Oscillator (HFO) , which happened just a few minutes ago.

There's no real problems for him to be on the HFO, which is actually gentler on the lungs than the conventional ventilator, but he cannot "graduate" to the C-PAP directly from the HFO, so he needs to spend some time on the conventional one first. Another "side-effect" of this change is that Antonio was starting to take some inhaling drugs (pulmicort and albuterol) which were supposed to help heal his lungs, and he cannot use inhaling drugs while on the HFO.

Also, they did another full blood panel, which showed a higher number of immature white blood cells. That could have been caused by the stress from the breathing issues, but it could also be a sign that he has some kind of infection starting, so Dr. Bennett also decided to restart him on antibiotics (for 3 days only). With the possibility of an infection, Dr. Bennett also decided to hold-off on starting him on steroids (to help develop his lungs), which is good so there’s no risk of him looking like Lou Ferrigno :o))
========

Another new video

We had another great visit today, and Dad got to change Antonio's diaper... I was scared to do it, b/c Antonio looks so fragile, that I was afraid that he would break. But I did it just fine and, amazingly enough, he did not "complain" and his o2 saturation did not drop!
I will be out for the next three days, working in NYC... I will miss him (and Sandra, of course) so much :0((

New Movie / Novo Filme

Just a quick post to upload this movie we got during our visit this afternoon...

The details are Antonio's hickcups, his chest wiggling b/c of the ventilator, and the "alarm" going off because his o2 levels were at 100% (ventilator at 41% only)!!!!

1/24/08 - NEWS

Well, they told us it would be a roller-coaster, and I think that is the best description of how our life has been for the past 17 days ... It's a good day after a bad day, and all over again! But Antonio keeps progressing, and in the end that is what counts!

Last night was another bad one... he seems to not like the nurses on the night shift,... actually, let me rephrase this: he likes one of his day nurses in particular -- Bridget. He is always on his "best behavior" whenever she is working with him, and always acts up for the rest of them! She's cute too, so I guess that Latin blood is still running in him...

The good news is that he is now being fed 4 cc of milk per hour, and he definitely has gained some weight -- they did weight him today and he is now 1 Lb 14 Oz (it is difficult to get an actual weight b/c of all equipment on and around him), but he does look more "meaty."

Other than that, he's still on the oscillator, and he has good days and bad days still... however, there is definitely no sign of infection in his lungs, but the problem is that he does not hold still, so the tube get dislodged quite often.

Well, that’s all for today folks!
================

Update 1/22/08

Good morning to all,

Just wanted to post a quick update on Antonio's progress. We visited with him yesterday afternoon and found that not much had changed -- except his size... He grew 3/4 of an inch and is now 13 inches long!!! It is too difficult to get an accurate weight though, because he is attached to so much equipment.

His breathing is still stable on the oscillating ventilator and they expect that he will be on it for a while longer (no timeline but they say as long as he needs it) before transitioning back to the regular ventilator and then to the CPAP.

They began his feedings again at 1 cc per hour and today have increased this to 2.5 cc per hour. He received another blood transfusion yesterday which is still normal for preemies because they do not have much blood and when they draw some for tests they have to replace it.

One of his nurses suggested that we get a "heartbeat bear" and take the heartbeat out to put into his incubator for calming him down after his care times when he gets so mad. The heartbeat bear has a little box in it that mimics the sound of a baby's mother's heartbeat when they are in the uterus because it tends to calm them down and remind them of more peaceful times. So he now has a heartbeat in his incubator.

The same nurse suggested that we start to get the dogs ready for Antonio by taking the sheepskin type fabric that is in his incubator home for the dogs to smell. So we brought it home twice and the dogs got so excited when they smelled it!!! Phoebe likes it so much that she sleeps on them as well :-).
==============================

Update 1/20/07

I am crazy busy at work today, so this update will be short, but sweet: all is good, and Antonio is doing much better. He's still not being fed, but getting enough nutrients via IV. A new X-ray hwoed that his lungs are doing much better on the Oscilator, and there was very little "yellow secretion" duirng his last suction.

So, this post will be just to show off our last visit... the shots are a little crappy, b/c I did not bring the camera and used the cellphone.

This is Antonio holding Dad's hand / Antonio segurando a mão do papai

Update 1/19/07

All is well! As we say in Brazil, nothing like a day after another... we just came back from visiting with Antonio at the NICU, and while last night's visit was distressing, today it was much better! Thanks to the wonderful staff taking care of him today, in special his nurse Gail... she also had twin preemies 20 years ago, so she was not only sweet, but also very helpful and gave us some great tips for down the road.

Also, I had a good conversation with a mother os two preemies (different births), who was giving some Kangaroo Care to her little daughter. Her experience realy helped me to get some confort today...

But enough about us, lets talk about our little guy: He is still in critical, conditions but very estable right now -- The ventilator settings came down to 35%, while he was requiring almost 100% O2 last night! He's still on the oscilator, but that seems to help him a lot, so he will be staying on it for a while. Dr. Benett did get an X-Ray of his chest, and it looked that it collapsed a little, but there was not infection... they did his blood work in the Am, and it was good, but in any case they started giving him some antibiotics, because the culture of his "suction" showed a little bacteria.

He got another bllod transfusion today, and that caused his bilirubin to go up again, but he's still our of the fototherapy for now, as they are still trying to not get him too stimulated. The great news is that he was very calm all day long, without the need to sedate him!

So this is it for now.... Sandra's dad and step-mom are leaving back to FL tomorrow morning, and Karin is cooking a nice German dinner for us, so I am going to help her a little.

Ciao!
=======================

Sua experiência me confortou bastante!

A Dra.

That's it folks!

Update 1/18/07

Well, yesterday I got to spend part of my birthday with little Antonio ... he could not eat cake, but I go to see his eyes again. He was out of the photo therapy as his bilirubin levels dropped considerably. All looked great, so it allowed me to go out and have some fun with friends and relatives....

But today, when we called in the morning to get an update, we heard that he had a rough night... his lungs are still not cooperating, and although he is stable, he's having a hard time with his O2 levels. The doctors changed his ventilator to a high frequency oscillator ventitalor, which is supposed to help with his breathing... and because he is a "wiggly worm" and always fights the nurses back, they had to sedate him so he would relax. More, they have his incubator covered, so he can be in the dark.... so after 10 great days, this had been our first tough day.

***********************************************

1/16/07 Update

We had great news and a great visit at St. Joseph today, and Antonio keeps doing very well. The only recurring issue is with him is still the lungs, which force the doctors to keep tweeking his ventilator, but he is improving ... Right now the ventilator is pumping at 46 bpm, down from 52 from last night, but we know that will be THE fight for the next few months.

The great news is that

  1. The results of the head ultrasound confirmed there was no bleeding in his little brain, and all looked perfect, and all ventricles looked normal as well

  2. They also did an echo of his heart to check for PDA (patent ductus arteriosu), and Antonio's little heart looked perfect as well

  3. They increased the volume of milk, and now he's being fed 2.5 cc/hour, and he did gain a little weight, about 2 Oz.

The best part of the visit was that Sandra was able to help the nurse during Antonio's "care" and she helped to change his diaper and took his temperature under his arm. He's always fusses when the nurses do that, but he took mommy's care without any complaints

************************************************************

Nós tivemos grande notícias e uma visita especial no St. Joseph Hospital hoje. Nosso pequeno Antonio continua est'avel e progredindo.

1/15/08 Update

Good morning to all,

Sorry for being so quiet, but the last few days since we got home have been a little crazy. Not only we had to get used to the new routine of daily routine, have fun with visitors and deal with phone calls, go to the hospital, etc, but also Sandra is still “tied” to the bed and I had to get back to work after three weeks… the inbox was cramped! Anyway, our little Antonio completed 8 days of age yesterday, and he sailed through the first week without any problems… here is the first update I post and it will include the latest developments: The only setback so far was that the ventilator tube got a little clogged two nights ago, which is something that can happen according to the doctors, so the CO2 level got a little high. But the doctors changed the settings to pump more O2 more often. First the BIG one: Yesterday, they did the ultrasound of Antonio’s brain to check for bleeding in the brain, which initially was one of the major issues concerning the doctors. Although we are still waiting for the final report, Dr. Bennett called yesterday to say that she did not see anything wrong.

We also got to touch him for the first time over the weekend; it was a bit of a scary experience but also the most wonderful thus far. They told us that preemies do not like to be caressed but rather they like it when you place one hand firmly on his head and the other on his legs or bottom depending if he is on his back or belly. He is so sweet and he really liked being touched by mommy and daddy.

His bilirubin levels continue to drop (4.0) and now he only has one light shining on him. Last evening during our visit the nurse gave him a break from the light and took off the protective bandage over his eyes so we could get a good look at his face and the little guy is so cute. He has opened his eyes already (he isn't supposed to do this yet).... Just goes to show that he truly is on his own timeline.

All the cultures on his blood came back and he has no infections so he is no longer on any antibiotic, but he's getting some caffeine to stimulate his breathing, and to keep with his origins!
The nurses are now feeding him and he is tolerating the food very well... he's getting 2cc of breast milk every hour for 11 hours, then 1 hour break and then 2cc per hour for another 11 hours and . He has lots of poopie diapers the nurses say and this is great because this is how babies eliminate the bilirubin in their bodies... so keep pooping Antonio!!!

*****************************

Sunday, January 13, 2008

I shot this video about 10 hours after Antonio's delivery, right after we had him baptized. It's sounds and looks like something out of a sci-fi movie... it's somewhat disturbing. But I can't tell you how emotional I got when I saw him moving his little arms...

Intro / Getting started

Well, this will be the first of various posts we will put up for the next months so family and friends can keep track of little Antonio's development ... we will try to keep them short and, hopefully, sweet .... and bi-lingual.
