What a Sunday... Que Domingão legal

There were so many good changes in Antonio's routine yesterday, that we will start with the not so great news: he either lost weight or, as we thought, there was a mistake when we weighed him on Saturday -- He was weighing 4 Lb 5.9 Oz yesterday. Other than that, all is great news...

Dr. Mike came in on Sunday and decided to start him on nasal cannula only. He did pretty well, and only had some minor bradycardia and bronchospasm episodes during the first day and night.



We got to spend most of the day with him on Sunday and it was clear that he was much happier with this new arrangement, so lets hope that he will stay that way. Another great development was that they tried to bottle feed him again... but this time he did really well.

We gave him 5 mL of milk, and he drank it almost in one gulp, smacking his little lips after was done, and looked around like asking for more. So, Dr. Mike suggested that we tried to give him another 5 mL and see how we would do.

Again, Antonio did a great job and got the extra food without any problem! And we got all that on video... See below.



Como vocês podem ver no vídeo acima, o Antonio conseguiu tomar sua primaira mamadeira ontem! Mas como neese post teremos muitas novidades, deixa eu começar com uma que não assim tão boa.

Nossa suspeita se concretizou, e quando pesaram o Antonio ontem ele mostou uma pequena queda, e portanto ele ou perdeu peso, ou realmente a balança não estava zerada quando o pesamos no sábado. De qualquer forma a diferença é assim tão grande, e ele está pesando 1.984 gramas...

Agora vamos às boas notícias: O Dr. Mike mais uma vez chegou com a corda toda para o plantão dele, e resouver antecipar as coisas e colocar o Antonio em 100% do tempo na cânula nasal. Ele reagiu muito bem às primeiras 24 horas, e apenas teve uns poucos episódios de bradicardia e broncoespasmo, nada mais nada!


Nós passamos a maior parte do domingo curtindo o filhote, e podemos dizer que ele realmente está gostando desse novo "regime"... ele estava mais bem alerta e mais ativo.

Agora à grande novidade, que foi o Antonio ter começado a mamar: As enfermeira já haviam tentado dar a mamadeira ao Antonio algumas vezes antes, mas ele não conseguia coordenar os atos de mamar, engolir e respirar, fazendo com que ele sempre tivesse uma ziquizira (i.e. bradicardia, broncoespasmo, desats...)


Mas dessa vez, foi a mamãe que deu de mamar pra ele, e ele tomou os primeiro 5 mL rapidinho, lambeu os beiços e ficou olhando de um lado pro outro querendo mais! Então o Dr. Mike falou pra tentarmos mais 5 mL, o que mais uma vez desceu redondinho pro Antonio!

Inacreditável is Unbelievable!

Hoje foi um dia especial para nós, pois algumas das roupinhas que haviamos trazido para o Antonio vestir na NICU já não cabem mais nele! Tá certo que eram roupinhas pra bebê prematuro, tamanho 0, mas elas estavam grandes nele há apenas algumas semanas. É emocionante não só ouvir que ele ganho peso, mas também ver a olhos nús que ele está crescendo dessa forma.

Agora, a parte inacreditável de hoje foi o ganho de peso que ele teve desde ontem - 84 gramas! Com esse ganho ele hoje estava pesando 1.998 gramas (veja abaixo a fotos dele na balança), mas logo antes de encerarmos a visita, ele "foi ou banheiro" e precisou de duas trocas de fraldas -- 18 gramas no total! Tô dizendo que esse menino aina vira presidente do bananal...

De qualquer forma, eu até acho que cometemos algum erro ou talvez a balança não estava zerada, pois pela progressão do peso do Antonio eu antecipava que ele iria atingir 2 quilos na quarta ou quinta-feira!

Tirando o peso, as grandes novidades é que ele deverá sair do CPAP de vez na segunda feira, uma vez que o Antonio está se dando muito bem apenas com a cânula nasal e já não aguenta mais aquele "tromba de elefante" presa ao nariz dele.

E pra terminar, o Antonio também está fazendo progresso no quesito alimentação. Hoje a enfermeira Eileen tentou um truque novo (que eu já havia sugerido muitas vezes antes!) e colocou a chupeta dele no leite, pra ele associar o gosto com o ato de chupar. Depois, quando a sandra tentou dar o peito pro Antonio, nós tanbém colocamos um pouquinho de leite bico do peito. Assim que o Antonio provou, ele não titubeou e começou a mamar com força!

No final desse post tem mais algumas fotos que tiramos ontem...


Today was a very especial day for us... Antonio outgrew his first clothes! Of course it was just some preemie onesies we got for him back when, but it is good to be able to actually see how much he is growing these days!

What was unbelievable was how much weight he gained since yesterday -- exactly 2.96 Oz! Now he is weighing 4 Lbs 6.4 Oz! You can see above what his weight registered on the scale (in grams). In any case, right before we came home, Antonio "had to go" a total of 18 grams, requiring TWO diapers to contain the damage he made...

Actually, I thought we made a mistake or perhaps the scale was not zeroed, because based on his progression so far, I only anticipated that he would reach the 2 kilos mark on Wednesday or Thursday.

Other than his weight, another piece of great news is that the doctors will try to take him off of the CPAP for good starting on Monday... that will be great, because he does very well on the nasal cannula and he hates that "elephant’s trunk" stuck to his nose!

And to finish, Antonio is also doing better regarding breast feeding... Nurse Eileen tried a different trick today (something that I had suggested Sandra a few times, by the way) that really worked -- she put some droplets of milk on his pacifier before giving it to him, so he can associate the taste with the "sucking motion". Then, when it was time to give him the breast, we also put some droplets of milk on Sandra's breast, which made Antonio go crazy and latch on almost immediately after he tasted the milk.

Quick update Rápido

Just to let you know that Antonio is doing great.

Today we got to give him another bath, which he loves... he now is being fed 33 mL of milk per feeding and he now weighs 4 lbs 3.5 Oz!

Apenas pra dizer que o Antonio continua progredindo bem... hoje nós demos mais um banho nele, e ele adora! Agora ele está recebendo 33 ml de leite e já está pesando 1.914 gramas!

Mais Boas Notícias & More Great News

Dá-lhe Garoto!!!

A Sandra acabou de me ligar pra avisar que nosso pequeno Antonio começou hoje mais uma "aventura" e vai alternar a cânula nasal com o CPAP, sendo a canula por SEIS horas e o CPAP por tres! Melhor ainda, que ele foi muito bem nas primeiras 6 horas que passou com a cânula!!!

Além disso, o Antonio deve ter estudado para mais um exâme de vista que aconteceu hoje, e ele foi bem melhor dessa vez! Os vasos sangüíneos ainda estão presente na retina agora na Zona II/III (aumentou a área, antes era Zona II), mas em grau 1 (antes grau 2).

O médico disse que, quando se vê uma melhora assim, quer dizer que se tem grande chances da ROP regredir totalmente... ele vai fazer um novõ exame daqui a duas semanas!

E pra terminar, o Antonio está pesando hoje 1.882 gramas.

Great & Fresh News!

Sandra just called me to let US know that today Antonio is starting a new adventure... he will start to alternate the nasal cannula and the CPAP, SIX hours on the cannula and three hours on the CPAP! Better yet, the first six hours with the cannula passed without any problems!

And more...apparently Antonio got mad about last week's eye exam results and decided to study for another eye exam he took today, as he did much better! There is still some progression and the doctor could see blood vessels on Zone II-III, but it was back to stage 1.

The eye doctor said that, when we see the eyes getting better like that, there is a good chance that the ROP will totally regress... he will check Antonio back in two weeks.

To finish, Antonio is weighing 4 Lb 2.4 Oz today!

baby is fat, mommy and daddy are happy! O bebê gordinho, mamãe e papai contentes!



More good news: As expected, our little guy passed the 4 lbs mark today, and is weighing 4 lbs 1.3 Oz!!! Also, the staff at the NICU asked us to be more active and participate more during Antonio's cares, which I believe and hope means they want Sandra and I to get ready... after all, Antonio's due date was supposed to be April 23!

Well, for us it seemed that we have been participating as much as they allowed us so far -- I still can't believe that was able to change his diapers and clothes while he was inside of the incubator -- afterall, I am a clumsy fella!

Anyway, I am more than ready and want to roll up my sleeves and do much more... The only issue are the times the cares are done in the NICU. Since both Sandra and I work fulltime, it gets a a little tricky for us to be there at the right time in the middle of the day... and if we get there late, we miss work and the opportunity of taking care of our little guy.

Mais boas notícias: Como era esperado, nosso pequeno Antonio passou das 4 libras e está pesando hoje 1.852 gramas hoje!!! Mais, o staff do hospitral quer que nós participemos mais dos cuidados higiênicos do Antonio, o que eu acredito e espero seja um sinal de que eles queiram que nós nos preparemos... afinal de contas era pro Antonio ter nascido em 23 de Abril, que esta chegando!

Nós achamos que estávamos participando ativamente em tudo que eles nos permitiram fazer até agora -- eu nem acredito que eu fui capaz de trocar as fraldas e roupinhas do Antonio com ele dentro da incubadora... eu sou muito atolado!!!

De qualquer forma eu estou pronto pra e quero arregaçar minhas mangas e fazer muito mais... o único probleminha é que tanto a Sandra como eu trabalhamos "fulltime" o que dificulta um pouquinho que nós conseguamos estar na NICU exatamente no horário que eles querem fazer os cuidados do Antonio... se nós chegarmos lá 5 minutos atrasados, é babau!

A enfermeira Lynn sempre brinca comigo perguntando se eu já quebrei o record mundial do maior número de fotos tiradas de um mesmo bebê, or se eu estou treinando o Antonio pra ser modelo fotográfico... Mas eu não resisto, pois não acho que o melhor registro do desenvolvimento do Antonio é o fotográfico...

Por exemplo, a primeira foto abaixo foi tirada momentos depois do Antonio ser trazido e estabilizado na NICU, e portanto ainda lutando pela sua vida. Já a segunda foto foi tirada no sábado, logo antes de ele ir dormir (dá pra ver pelo bocejo!)... não é impressionante a mudança em "apenas" 76 dias?



Nurse Lynn always jokes with me asking how many pictures I take of Antonio and if I already have broken Guinness record of more pictures of one single baby, or if I am training Antonio do be a model on GQ... but I can't resist, because I think that the best way to show Antonio's development is through the images.

As an example, the first picture above was taken moments after his birth and after he was brought into the NICU, and still fighting for his own life. The second picture was taken on Saturday afternoon, right before he went to sleep (as you can notice by the yawning) and you can really see how much he changed in "only" 76 days!

So there you have it... how can I not go for the pictures, when I know they will be as good as the ones below, taken on Easter Sunday?

Weight Chart / Acompanhamento do Peso

Just a quick post to let you know that Antonio is doing great with the last changes in his diet, and today he is weighing 3 Lbs 15.2 Oz mark... hopefully tomorrow he will get another "certificate" for passing the 4 Lbs mark, unless he loses much on his next diaper change!

Apenas um post rapidinho pra avisar que o Antonio está se saindo muito bem com as últimas mudanças na sua dieta e hoje estva pesando 1.792 gramas. Se continuar assim, amanhã ele receberá mais um "certificado" por ter passado de 4 libras (1.815 gramas), a não ser que ele perca muito peso na próxima fralda!

ps: Nurse Melissa thinks that he was having a growth spurt today, because he was tired and sleepy for most of her shift...

ppss: A enfermeira Melissa nos disse que ele deve estar dando uma arrancada no crescimento hoje, pois o Antonio estava muito sonolento e cançadinho durante a maior parte do plantão dela.... Deus a ouça!

Boa Noite & Good night!

Como prometido, acabamos de ligar por hospital, e esses são os dados:
  • Peso: 1.770 gramas, oficialmente dobrando o peso que ele nasceu (875 gramas)

  • Altura: 40 cm -- o mesmo que na semana passada

  • Cabeça: 30 cm de circunferência -- o mesmo que na semana passada

Eu estou um pouco surpreso com o tamanho as medidas que me deram pelo telefone, pois o Antonio está parecendo bem maior do que na semana passada! Talvez seja apenaS pelo peso ... De qualquer forma, com o aumento de peso, apartir de hoje ele começará a receber 31 mL de leite a cada alimentação.

As promised, we just hung up with the hospital and here is the scoop:
  • Weight: 3 Lbs 14.4 Oz, officialy doubling his birth weight (1 Lb 15 Oz)

  • Lenght: 15 3/4 inches -- the same as last week

  • Head: 11 3/4 inches around -- the same as last week

I have to confess that I am a little surprised with his "measurements" as he looks much bigger than last week! It must be the weight gain that makes him looks fuller... in an case, with the new weight, Mr. Antonio gets to eat 31 mL of milk in each feeding.

Growing... Crescendo!


What can I tell you... Antonio is really doing a great job growing!

The doctors consolidated his feeding a little more a few days ago, and he's still getting the same 29 mL, but now over 45 minutes and then resting for 3 1/4 hours. He's is filling out very nicely and he almost doubled his birth weight, weighing today the whopping 3 Lb 13.2 Oz!

He is still needing the CPAP, but he is alternating with the nasal cannula every three hours, and doing great! I believe that some time next week they will increase the time he spends on the cannula.

Now, the best of all is that he is really very alert and interacting with people and things (see below), giving us little smiles here and there, that simply melt our hearts... I know that I am biased, but he's getting to be a cute little guy!


I mentioned before that Sandra and I get to decorate Antonio's crib, and the other day we brought in this mirror -- a suggestion given by Nurse Gail. We thought that it would not get used at all, but today Nurse Jane put it up and told us that he actually spent almost two hours looking at the "other baby"!

Also, he got really fond of his pacifier as you can see on the video below. That is great because it is good training for breast feeding. The background voice is Sandra telling him a story on his recorder.




Como você pode ver no vídeo acima, o Antonio aprendeu a gostar da chupeta dele, o que no case dele é muito bom por que serve como treinamento pra ele aprender como se alimentar no peito.



Eu nem preciso dizer nada... O Antonio está crescendo e ficando gordinho a cada dia que passa. Os médicos consolidaram mais uma vez a alimentação dele, e agora o Antonio está recebendo os mesmo 29 mL de leite, mas agora em apenas 45 minutos, descansando por 3 horas e 15 minutos. A nova dieta está realmente surtindo efeito e ele quase dobrou o peso do nascimento, pesando hoje 1.735 gramas!

Ele ainda está recebendo o CPAP, mas continua alternado com a cânula nasal à cada 3 horas e está se dando muito bem. Eu acredito que na próxima semana eles devam aumentar o tempo que ele passa na cânula nasal.

Agora, o melhor de tudo é que o Antonio está ficando mais alerta a cada dia que passa, cada vez mais interagindo com as pessoas, e dando uns sorrisos lindos que nos nos amolecem o coração. Eu sei que sou suspeito pra dizer qualquer coisa, mais o moleque está ficando bonitinho! E vaidoso...



Como eu havia falado nós podemos decorar o berço do Antonio e, seguindo uma sugestào da Enf. Gail, trouxemos esse espelho há alguns dias... a princípio achávamos que não seria aproveitado, mas hoje a enf. Jane o colocou no berço e nos falou que o Antonio passou quase duas hora olhando pro "outro bebê".

One week of waiting / Uma semana de espera

Oh well, as always, not all news are good... Antonio had a follow up eye exam last night, and the doctors said there was a progression on his ROP and he could see some growth of abnormal new blood vessels in the retina, Zone II, stage 2 according to the classification out of three zones and 5 stages.

Those abnormal blood vessels may regress, but frequently they progress, and although Stages 1 and 2 do not lead to blindness, they will need to keep an eye (no pun intended) on Antonio's eyes. So instead of every two weeks, the doctor will come next week again to evaluate his eyes again, in case he needs surgery.

There are many treatments for ROP, including the destruction blood vessel using laser or cryotherapy, both which can be performed right there at the NICU without any major problems.

In case you wonder (again, no pun intended) Stevie Wonder is a famous person who has the disease...

Como sempre, se tratando de um bebê prematuro, as noticias não podem ser boas o tempo todo. O Antonio teve mais um exame de vista na segunda-feira, e os médicos constataram que a ROP progrediu, e eles puderam ver vasos sanguíneos na retina classificados zona II Grau, numa classificação de três zonas e 5 graus.

Esse vasos podem regredir, mas infelizmente ele com frequência continuam a progredir, e mesmo que nos estágios 1 e 2 nào podem produzir a cegueira, os médicos vão intensificar os examas e ou invés de faz6e-los a cada 2 semanas, eles começarão a vê-lo todas as semanas, em case o Antonio precise de cirurgia.

Existem vários tratamentos para a ROP, incluindo a destruição dos vasos através com o uso de laser e crioterapia, e ambas podem ser feitas lá mesmo na NICO sem maiores problemas.

Just a quick one... Um rapidinho

Just to let you know that Antonio is doing very well with all the changes... With the exception of a few hickups - like in a major bradycardia episode on mommy's arms this afternoon! -- he is improving both regarding the feeding and the breathing.

He's now up to 3 lbs 10.5 Oz, and getting chubbier every day, and he's liking the new one-hour-in-three-hours-out diet. Other than a few spit ups, there were no major issues so far.

About breathing, Antonio is actually alternating the the nasal cannula and the CPAP for three hours each throughout the day. He also is doing well on this new regimen, and his sats were good for most of the day.

Apenas um post rapidinho pra dizer que o Antonio está se dando muito bem com todas as mudanças... com algumas pequenas exceções -- como um episódio de braquicardia daqueles no colo da mamãe durante a nossa visita da tarde -- o Antonio está melhorando tanto no quesito respiração como no de alimentação.

ele agora está pesando 1.656 gramas e está ficando mais gorduchinho a cada dia. ele está gostandop da dieta de uma-hora-sim-três-horas-não e só teve pequenos acidentes.

Em relação à respiração, o Antonio na verdade está desde hoje alternado entre a cânula nasal e o CPAP por três horas cada durante todo o dia. Ele também tem se saido bem nesse regime e seus sinais vitais se mostraram, estáveis durante o dia todo.

So many changes... Quantas mudanças!

Bem, é melhor você se sentar pois esse post será longo... O Dr. Mike chegou com a corda toda do fim de semana e fez mudanças radicais (que eu já estava esperando!) na rotina do Antonio.

Vamos começar pela mais fácil de entender: O Antonio estava instalado numa incubadeira normal desde o fim de fevereiro, mas eu estava achando que ela estava ficando pequena pro Antonio -- Aliás algumas enfermeiras também já haviam comentado várias vezes que tava na hora de mudarem ele para um bercinho...

Então hoje pela manhã eles resolveram graduá-lo para um berço... berço, não berção -- por causa dos equipamentos que o Antonio ainda precisa, eles resolveram colocá-lo num berço infantil da pediatria para dar mais espaço.


Eu sei que é bem feinho o berço, parece até uma jaula de zoológico, mas nós podemos enfeitá-lo um pouquinho! De qualquer forma, para nós isso não é importante... por mais que eu me esforçasse, não descrever o quão feliz a Sandra e eu estamos por essa pequena mudança de ambiente. Não só por que agora é muito mais fácil acessá-lo, mas principalmente por que isso significa que o Antonio já consegue manter sua temperatura, o que é uma das coisas mais importantes que ele precisa superar antes de poder ir pra casa!


Uma outra coisa que ele precisa superar -- ou aprender -- é a se alimentar pela boca. Nesse quesito, os médicos consolidaram pela última vez a alimentação do Antonio, e desde ontem ele está recebendo 27 mL de leite por uma hora e tem três horas de "descanso", o que é o regime "normal" para um recem nascido.

Até agora ele está indo bem nesse novo regime, mas o número de vezes e quantidade de leite que regurgita aumentou bastante, devido ao estômago dele ainda ser pequenino e ainda precisa se acostumar com o volume maior de leite que ele está recebendo.

Quanto ao último quesito necessário para o Antonio vir pra casa -- respirar por conta própria -- também desde ontem ele voltou a receber apenas a pressão de ar contínua (CPAP) e não mais nenhuma respiração adicional pelo ventilador. Além disso, hoje pela manhã o Dr. Mike também resolveu tentar a cânula nasal mais uma vez... daqui a pouco eu vou ligar pro hospital e ver como ele se saiu.

De resto, como coloquei no post de ontem ele agora está com 40 cm de comprimento e pesando 1.604 gramas... a cabecinha dele está com 29 cm de circunferência.


Well, it is better you to sit down because this will be a long post... Dr. Mike arrived from the weekend in good mood and decided to make several radical changes (that we were waiting and hoping for!) to Antonio's routine.

So, let's start with the easiest one to explain: As you know, since February 29, Antonio's living quarters was a regular incubator, but lately I was thinking that it was getting a little too cramped for him in there -- some of the nurses had also commented that it was time for him to move on to the bassinette...

So we were happy to hear that yesterday morning they decided to graduate him and he gets to "live" in a cradle... but not a little cradle, a large one! (see the pcitures above). Because of all the equipment that Antonio still requires, they decided to place him in pediatric cradle so he can have more space.


Yes, we know that the cradle is quite ugly, even that it looks like a zoo cage, but Sandra and I can decorate it a bit ... and by all means, that is not important at all for us! Because, no matter how hard I try, there is no way for me to explain how happy Sandra and I are for this small environmental change. Not only becuase it is much easier to have access to Antonio, but mainly becuase this change means that he is able to maintain his temperature, and that is a major milestone he needed to achieve before being able to come home!

Another milestone for him is regarding his feeding, as he needs to "learn" to be fed normally by the mouth. To get him in that direction, the doctors consolidated his feeding for the last time yesterday, and now Antonio is receiving 27 mL of milk for one hour and has three hours of "rest," which would be the "normal" regimen for a newborn.

So far, so good and Antonio is tolarating well his new diet, but the number and volume of his spit ups increased, since his stomach is teeny and needs to get used with the higher volume of milk that Antonio is receiving.

The last and the more complicated issue we need to solve before Antonio gets the green light for him to come home is for him to breathe on his own. So to get him ready, also since yesterday, he is back to receiving only the CPAP and no more additional breaths by the ventilator. In addition, since this morning, Dr. Mike also decided to try the nasal cannula for four hours a day.... in a little bit we will call the hospital to check how he is doing on that.

And to finish, as I posted yesterday, Antonio is now 15" 3/4 long and weighs 3 lbs. 8.6 oz... his head is 11" 3/4 around.

Our boy is growing up and is now 15" 3/4 long, weighing 3 Lb 8.5 Oz -- I am sure that in a few weeks he will be asking for the car keys... Today there will be many changes on his daily routine, and I will tell all about it on a separate post that will come later.

Nosso pequeno Antonio está crescendo rápido, e ontem estava medindo 40 cm e pesando 1.604 gramas... tenho a certeza de que em muito pouco tempo ele estará pedindo a chave do carro pra dar umas voltas! Hoje os médicos farão algumas muitas mudanças na rotina diária do Antonio, as quais precisam de um post específico que virá mais tarde.

Antonio e a vaca / Antonio and the cow



Hoje nós resolvemos trazer uma "velha amiga" pra visitar o Antonio -- a vaquinha que era a companheira dele durante as longas primeiras semanas de NICU. É a mesma vaquinha que ele está segurando no topo da página, e a foto acima dá uma boa idéia do quanto ele já cresceu...

Confirmando o que já haviamos falado no post anterior, os médicos consolidaram a alimentação do Antonio, e ele agora está recebendo leite à razão de 17,4 mL/hora por duas horas seguidas, e depois tem um break de duas horas. Ele começou a receber essa nova dieta ao meio dias, e até agora está reagindo bem.

Quando chegamos no hospital hoje, ele estava sendo retirado da balança, e o moleque está ficando gorduchinho e com dobrinhas (veja foto abaixo), pesando 1.574 gramas! De resto nós curtimos a uma visita, com o Antonio alternando entre os braços da mamãe coruja e do papai babão (como diria o Tio Ronaldo Derly), sem reclamar muito.



As you can see from the picture on the top of this post, today we decided to bring an "old friend" to visit Antonio -- the little beanie baby cow was his companion during his long first weeks in the NICU. That is the exact same cow Antonio is holding in the picture at the top of the page, and that gives you a good idea of how much he has grown since then...

Confirming what we said on the earlier post, the doctors consolidated his feedings , and now Antonio is receiving milk at the rate of 17.4 mL/hour for two hours, taking a break of two hours after that. He started this new diet at noon, and so far is is doing fine.

When we arrived at the NICU, Antonio was being removed from the the scale, and the little guy is getting to be a piglet, complete with baby rolls (see the second picture above), and he's weighing 3 Lbs 7.5 Oz! He spent the rest of the afternoon between the mommy and dad's arms, without complaining too much.

Saturday update de Sábado

Hello folks... just another quick update. Antonio continues doing great on his new diet and was weighing 3 Lbs 6.6 Oz yesterday.

So today they will move him up to the next step: He will receive his feedings for 2 hours and rest for another two hours. I am not sure how much he will be geting per hour, so I will add this info here later.

That's all for today... we are going to Maya's birthday (Ryan & Abby's daughter) party, and later we will go to the hospital. We will post any news later tonight.

Bom dia a todos! Apenas um post rapidinho pra dizer que o Antonio continua se dando bem com a nova dieta e ontem já estava pesando 1.546 gramas.

Portanto, hoje os médicos resolveram consilidar a alimentação mais uma vez e passarão a alimentá-lo por duas horas com duas horas de descanço... Ainda não sabemos o volume que estão dando pra ele, então voltarei aqui para colocar essa informação mais tarde.

De resto está tudo bem.. nós estamos indo à festa da Maya (a filha dos nossos vizinhos Ryan e Abby) hoje, e mais tarde vamos ao hospital. Qualquer novidades eu postarei hoje à noite.

No News is Good News

Só pra dizer que, tudo está correndo bem, e o Antonio está reagindo de forma maravilhosa à nova dieta -- ele acabou de ser pesado (20:00) e está com 1.543 gramas.

A Dra. Bennette disse que amanhã provavelmente vão tentar usar a cânula nasal novamente... e se tudo dar certo vão recomeçar a usá-la todos os dias por 4 horas.

Just to say that all is well... Antonio is doing great on the new diet, and they just weight him (8pm) and he is 3 Lbs 6.4 Oz today!

Dra. Bennett said that perhaps they will try the nasal cannula tomorrow again.. if he does well, then they will restart it for 4 hours each day.

Oh well, "for good" now means "about one hour" ... Antonio did not react very well to the changes in the ventilation, and he's back to the SIPAP. It's not a big set back, but in any case it frustates us a little, as the two major changes in that area(no SIPAP and the use of the cannula) were discontinued for now.

In any case, at least regarding feeding, Antonio is doing very well. He likes the new regimen that consolidates his feeding, so the doctors are sticking to it. Yesterday his weight was 3 Lbs. 3.8 Oz.

Other than that, no news...



Hoje em dia, "permanentemente" quer dizer "mais ou menos uma hora"... parece até contrato de jogador de futebol! O Antonio não reagiu muito bem à mudança feita no ventilador ontem, e voltou ao SIPAP. Não foi nada de mais, mas uma "recaida" que nos frusta um pouco, pois essa foi a segunda das grandes mudanças (SIPAP e cânula nasal0 que foi descontinuada esta semana!

De qualquer forma, o Antonio está reagindo bem em relação à nova dieta... segundo as enfermeiras, ele gosta de receber a alimentação consolidad, então os médicos vão continuar dessa forma. Omtem ele estava pesando 1.468 gramas.

De resto, tudo bem...

Mais mudanças / More Changes

Quem te viu e quem te vê, Antonio... Agora você já está um menino crescidinho e ficando tão saudável que os médicos não tem medo de tentar coisas novas a cada dia.

As novidades para hoje são, primiramente, que o Antonio está, PERNAMENTEMENTE, fora do SIPAP. Ele ainda está recebendo oxigênio através do CPAP, que mantém uma pressão continua no pulmão para evitar que ele colapse e também que o Antonio entre em apnéia. A máquina não força mais nenhuma respiração por minuto (o Antonio recebia 10 rpm até hoje pela manhã).

Essa mudança só ocorreu por volta das 9:00, então ainda é cedo pra ver como ele via reagir. A Enf. Virginia que está tomando conta dele hoje me disse que normalmente os bebês precisam receber mais O2 quando fazem essa mudança, mas nem isso o Antonio precisou.

A outra mudança foi em relação à alimentação: Os médicos começaram a consolidar a alimentação do Antonio, que agora prssará a receber o leite materno à razão de 11 mL/hora por três horas, e descansará por uma hora -- antes ele estava recebendo o leite durante 11 horas, com uma hora de descanso. O leite ainda está sendo dado através da sonda.

Mais uma vez, ainda é cedo pra julgar como ele vai receber essa "dieta", mas esse é o caminho pra ele começar a se alimentar normalmente. Então ele ficará nesse regime por alguns dias, e depois os médicos vão aumentando o volume e diminuindo o intervalo da alimentação, até ele começar a receber leite por uma hora e descansar por três.



Antonio, you are a big boy now... you are doing so well that the doctors are not afraid of trying new things every day.

The great news for today is that, first, Antonio is out of the SIPAP for good. He is still on the CPAP to keep his airway open under air pressure, and reducing the risk of the lungs collapse and of apneas. The machine still keep some O2 pressure, but is not giving him any more breathes per minute -- he was getting 10 bpm until this morning.

This change was made around 9am, so it is still early to tell how Antonio will react. Nurse Virginia told me that most of the time the babies need to get additional O2 when a change like this is made, but not even that was required for him... it's a good sign.

Another big change is related to his feedings: The doctors decided to start consolidating his feedings, and Antonio is now receiving milk at the rate of 11 mL/hour for three hours, then he will have a 1 hour break and start all over again.

Again, it is still early to tell how he will react to this new "diet," but that is the plan to get him to eat normally... so he will be getting this regimen for a few days and then they will increase the volume of milk and reduce the time of the feeding, until Antonio starts to eat for one hour and rest for three.

We just got this picture from the nurses today... Antonio is all dressed up, ready to party! Guess who was his nurse? He's such a Latin Lover....

His weight today was 3 lb 3 Oz.



Nós recebeos essa foto das enfermeiras hoje... O Antonio está todo arrumadinho, se preparando pra festa, pra chegar na festa todo arrumadinho. Adivinha quem era a enfermeira hoje? Esse é safado que nem o pai...

Ele está pesando hoje 1.444 gramas.

3/10 Update de 10/3



Go figure, when we think there will be no news for a while, we got a big surprise yesterday: Antonio got to spend a few hours off of the SIPAP and use a regular Nasal Cannula to get his supplemental oxygen!

The doctors decided to try the cannula for four-hours a day, and he did so well on his first session that the nurses did a second session during the night. Again, he did well, but a blood gas early this morning showed that the level of CO2 (69) was a little elevated, so they will stop using the cannula for a few days. The blood gas they ran at noon already showed the CO2 level at normal rage (45).

Also, he got very interested in the breast yesterday, and he even got to suck hard enough to get some milk flowing... he has definitely been on his own schedule since the beginning!

Antonio lost a little weight and was at 3 Lbs 1.3 Oz, but it is normal for his weight to fluctuate on a daily basis -- it is important for him to keep a weight gain trend as shown in this chart. He is being fed at a rate of 9 mL/hour.

Vamos começar falando sobre o peso, uma vez que o Antonio perdeu um pouquinho ontme estando com 1.400 gramas. Essa flutuação de peso é normal, e não devemos nos preucupar com ela e sim focalizar na tendência mostrada no gráfico ao lado. Ele agora está recebendo 9 mL de leite por hora.

Agora, o Antonio está cada vez mais mostrando melhoras e ontem nós fomos surpreendidos em v6e-lo usando a cânula nasal pela primeira vez para receber as "doses" extras de oxigênio.

Os médicos resolveram inicialmente deixá-lo usar esse aparato por quatro horas todos os dias, mas ontem depois da primeira sessão o Antonio etava tão bem que o daixaram por mais 4 horas durante a noite. Apesar de não ter tido nenhum problema, uma gasometria pela manhã mostrou o nível de CO2 um pouco elevado (69%), e com isso resolveram para com a cânula por alguns dias. A gasometria do meio-dia já mostrou CO2normal (45)

E pra terminar, e mostrar o quanto ele está no seu próprio tempo, o Antonio ontem se mostrou bem interessado em relação ao seio, passando bastante tempo "brincando" e até mamou com força suficiente para tomar um pouquinho de leite!

Agora, a única coisa que eu peço é que eles dêem um jeito na cabecinha do Antonio... pois. por ele ficar deitado por mais de 60 dias a cabeça dele está um pouco "quadrada" e ele está parecendo com o Paul Giamatti! Não achas?

After two day of "darkness" / Após dois dias de "escuridão"

Desculpa o silêncio, mas estavamos sem cabo-telefone-internet desde sábado, e portanto sem condições de "postar" qualquer novidades... de qualquer forma, tudo está bem com o Antonio desde então.

A única grande novidade é que ele continua crescendo, e está medindo agora 39.5 cm, e sua cabeça 28.5 cm de circunferência -- bom sinal de que o cérebro está se desenvolvendo bem. Além disso, o Antonio está pesando 1.425 gramas e passou a receber 8.5 ml de leite por hora.



Sorry for the "silence," but we were with no cable, phone or internet since last Saturday, so it was impossible to post any news. In any case, all is well with Antonio.

The only great news is that he keeps on growing and is now 15 1/2" long, and his head is 11 1/4" around -- a good sign that his brain is developing. Also, he's now weighing 3 Lbs 2 Oz, and being fed at the rate of 8.5 mL/hour.

Happy Birthday Mr. President....



Antonio is today (60) days old, and getting a little bigger and plumper each day. He is now 2 Lb 15.7 Oz, and was introduced to the breast for the first time (no pictures, you pervert!). He's definitely my son, as it did not take much encouragement for him to start playing with those puppies!

Oh well... we would not post the picture below, as the lighting makes our little boy look like something out of a horror movie -- Alien or Carrie or something like that -- but at the same time his face and body expressions are priceless. That was taken during his first actual bubble bath, so it's like he is saying " What are you doing to me... I DON'T LIKE BUBBLES!!!" By the way, reddish tint is from the heating light.

O antonio está completando -60 dias hoje!!! Ele está crescendo e ficando mais rechonchudo a cada dia que passa. Ontem estava pesando 1.351 gramas e foi apresentado ao peito pela primeira vez... ele é realmente meu filho, pois o bichinho sapeca foi lá, de primeira, e botou o bocão!!!! Não colocaremos nenhuma foto não, seu tarado!!!

Nós não íamos colocar a foto ao lado, pois a iluminação o faz parecer como algo saído de um filme de terror -- Aliens ou Carrie, A Estranha -- mas ao mesmo tempo, as expressões faciais e corporais dele são impagáveis!! A foto foi tirada durante o primeiro banho de espuma que ele tomou, e é como se ele estivesse falando "PQP, o quê vocês estão fazendo comigo... Eu odeio espuma!!!!"

Ah, a luz vermelha é do aquecedor que colocam sobre ele quando está fora da incubadeira.

De resto, eu agradeceria a todos se deixassem mensagens pro Antonio aqui no blog... acredito que ele adorará lê-las quando estiver crescidinho. Basta clicar no "Comments" abaixo.

Notícias fresquinhas... Fresh news!

Primeiramente, deixa eu gritar: Neeeeeeeense!!!! Esse Antonio é pé quente mesmo! 6 x 0, com direito a 3 golaços contra os hermanos... Neeeeeeense!!!!!

Pronto, extravazei a euforia, e agora vamos falar do Antonio. Pra começar, uma correção: a hérnia que ele tem não é abdominal e sim inguinal. Apesar de não ser perigoso, os médicos e enfermeira continuam observando, para que possam evitar qualquer complicação caso a hérnia fique estrangulada, o que requeriria cirurgia de emergência.

Tirando esse problema, o progresso do Antonio está indo de vento-em-popa: ontem passei boa parte da tarde com ele, e o segurei por quase uma hora (fotos e vídeos abaixo)! Durante a maior parte do tempo, tanto a respiração quanto os batimento cardíacos do Antonio estavam em níveis excelentes, levando a enfermeira a colocar o SIPAP pra funcional com apenas 20% de O2 e 18 ventilações por minuto... Mesmo assim, ele estava com 95-98% de saturação de O2! Além disso, hoje recebemos um excelente feedback da Dra. Bennett, e vão fazer muita mudanças no "regime" do Antonio:

  1. Primeiramente reduziram ainda mais o número de ventilações por minuto que SIPAP está dando ao Antonio, agora para 10 vpm. Com essa redução, estão precisando dar mais O2 pra ele, mas o SIPAP esta ventilando a apenas 33% neste momento.

  2. Descontinuaram de vez o uso do Albuterol e da Aminofilina (Aminophylline), e vão recomeçar a dar cafeina para ele. Os médicos acreditem que com a retirada daqueles dois medicamentos, a cafeina não deva aumentar tanto a frequ6encia cardíaca do Antonio.

  3. Moveram a sonda da alimentação, recolocando-a no estômago (antes estamna diratamente nos intestinos) o que vai dar a ele a sensação de estar com a barriguinha cheia. Acho que isso é para cmeçar a treiná-lo para a amamentação, uma vez que também vão deixar ele "chupetar" no peito uma vez por dia.

  4. Ele voltou a ganhar peso, e ontem estava com 1.310 gramas!

Let me start by saying: NEEEENSE! NEEEEEENSE!!! Antonio is really the new lucky charm for Fluminense's crowd! We won 6 x 0 against the "hermanos", being that 3 of the goals were, simply put, spectacular! Neeeeeeense!!!!!

OK, so with that said, we can start the actual post for today: First a correction, as Antonio's hernia is not in his abdomen but is an inguinal hernia. Although it is not dangerous, the doctors and nurse continue to check it throughout the day, so that they can prevent any complications in case that the hernia gets strangled, that would require an emergency surgery.

Other than this problem, Antonio's progress is great: yesterday I spent good part of the afternoon with him, and I held him for almost one hour (photos and videos above)! During most of the time, his heart rate and breathing rate were excellent, which made nurse Cathy reduce the O2 being pumped to 20% even though the SIPAP was only giving him 18 bpm! Moreover his O2 sats were between 95-98% the entire time!

Today, we received excellent feedback from Dr. Bennett, and they went already and made several changes to Antonio's "regimen":


  • First they reduced even more the number of breaths per minute given by the SIPAP, now to 10 bpm. With this, Antonio is requiring a little more O2, but the SIPAP setting is still at only 33% at this moment.


  • They discontinued for good the use of Albuterol and Aminophylline , but started giving him some caffeine. Doctors believe that, with the discontinuation of those two medications, the caffeine should not affect Antonio's heart rate.


  • They repositioned the feeding tube to Antonio's stomach (before is was directly into his intestines), so that might give him the sensation of having his belly full. I believe that is to start training him for breast-feeding, since they will also let him nuzzle on Sandra's breast once a day.


  • He also showed good weight gain yesterday, now weighing 2 Lbs 14.2 oz!

3/5 Update de 5/3

I was in NYC on Monday and Tueday, so it was a little difficult to post any news... also, there were not so many developments since I left on Monday morning. In any case, since he came off of the ventilator, all is status quo with Antonio. However, there is a couple of issues:


  • Heart rate: His heart rate has been quite high, which was weird since he actaully had shown very slow rest rates (around 100 bpm). In any case, yesterday his heart rate was on the 200s, reaching as high as 220 bpm ... Dr. Bennett checked and eliminated all the other possible causes for that (low hematocrit,high temperature, high CO2 levels), and she believes that is was the Albuterol that was causing it. So, she put that drug on hold since last night, and his heart rate is stable at the 180s.



  • Since last week, his weight gain stopped and he's still at 2Lb 12 Oz. Antonio's weight has been up 5-10 gram one day, and down 5-10 gram the next... The doctors believe that is because of the change from the ventilator, demanding more from him, so he's burning more calories than before, but in any case my understanding is that making him gain weight will be the fight.


  • Hernias: We noticed that he developed hernias, which are commom for premmies. In any case, he will require surgery later on, but they will start doing daily exams. Right now, they are “retractable” which means they are soft and pink and not painful. If they are hard and blue and painful they would need to do an emergency surgery so that is why they check them now.



Other than that, Dr. Bennett mentioned that his blood gases are great, and that his O2 sats are good. She also mentioned that she listened to his lungs and they sounded clear.

Eu estava em Nova York na segunda e terça, então ficou um pouco mais difícil de postar... além disso, não temos tantas vovidades desde que o Antonio saiu do ventilador no sábado e tudo está estável... porém temos dua preocupações:



  • Frequência Cardíaca: Ontem a frequência cardíaca do Antonio estava um pouco alta demais, chegando ao 220 bpm. Dra. Bennett eliminoou todas as outras possíveis causas para isso, e chegou a conclusão que deveria ser efeito colateral do Albuterol. Portanto eles pararam de dar esse medicamento pro Antonio desde ontem à noite, e agora a frequencia cardíaca está nos 180s.

  • Peso: Desde a semana passada quando atingiu 1.286 gramas, o Antonio parou de ganhar peso'... ele ganha 5-10 gramas hoje, e perde o mesmo peso amanhã. Os médicos acreditam que seja resultado do espforço extra quye eles tem feito desde que saiu do ventiladopr, o que requere que ele queime mais calorias. De qualquer forma, acredito que o ganho de peso será a próxima batalha que o nosso pequeno Antonio terá que enfrentar.



  • Hérnias: Nós notamos que ele desenvolveu hernias no abdomen, o que é comum em bebês prematuros, então ele vai precisar de cirurgia mais tarde. De qualquer forma, vão começar a fazer exames diário pra chacar o estado das hernias, que por enquando se apresentam macias e rosadas, o que significa que ele não sente dor. Caso elas estivessem azuladas e firmes, será preciso fazer uma cirurgia de emergência.

De resto, tudo está estável... a Dra. Bennett aucultou o pulmão do Antonio hoje cedo e nos disse que parace claro e limpo!

SIPAP, the return of the Nose Prongs

First, let me clarify that Antonio was put on the SIPAP, and not on the CPAP (which have already happened before) as we originally mentioned -- for some reason, I got confused between the two due to some information I got from doctors when Antonio was born... Anyway, he has been on the SIPAP since yesterday morning, and doing great! (CONTINUE AFTER THE JUMP)

The sipap machine is working on very low settings (O2 Sat set at 21-26%), and Antonio is getting "room air" (21% O2) for long periods of ime during the day. All his blood gas tests were great, with CO2 levels at 40-45%.

Tonight he will get his last dose of Decadron, which has helped him considerably on the transition from the conventional ventilator, so Monday will be day for us to confirm that he turned the corner regarding the breathing issues.

We actually have not much more to say, only that we heard his little voice for the first time -- not really his voice, but his cries. He's still a little hoarse from the ET tube, but that did not stop him from crying a little when the nurse was doing something he did not like.

Ah, and he now is getting fed at the rate of 7.6 mL/hour. We have some pictures and videos of our last two visits posted below... as you can see, even with the change we were still able to hold him for almost 1 hour each day... Enjoy!

Primeiramente, gostaria de fazer uma correção: O Antonio foi colocado no SIPAP e não no CPAP (o que já havia ocorrido anteriormente). Eu me confundi com algo que os médicos nos disseram quando do nascimento do Antonio... de qualquer maneira, ele está no SIPAP desde ontem demanhã, e está muito bem obrigado!

A maquina está funcionando com "settings" bem baixos (saturação de O2 há 21-26%), e o Antonio está recebendo ar-ambiente (21% de O2) por períodos prolongados durante o dia, e mesmo assim os exames gasométricos estão muito bons com os níveis de CO2 entre 40-45%.

Hoje à noite ele receberá a última dose of esteroide decadron, que o ajudou imensamente nessa transição do ventilador ao sipap, e portanto amanhã será O dia para confirmarmos que o Antonio realmente saiu daquela fase mais problemática da sua respiração.

Na verdade, não temos muito mais há dizer, apenas que ouvimos a vozinha dele pela primeira vez -- não a voz de verdade, mas seu choro... ele ainda está meio rouco por causa do tubo endotraqueal, mas mesmo assim ele dá uma choradinha quando a enfermeira fazia algo que ele não gostou muito.

Ah, ele agora está sendo alimentado 7.6 mL de leite por hora, e ele está começando a ficar com uma bochechinha linda. Abaixo nós estamos postando algumas fotos e vídeos da nossas últimas visitas. Como vec6es poderão ver, mesmo com a mudança, nós aida podemos segurá-lo por quase uma hora em cada dia. Beijos & Abraços!
