Mais mudanças / More Changes

Quem te viu e quem te vê, Antonio... Agora você já está um menino crescidinho e ficando tão saudável que os médicos não tem medo de tentar coisas novas a cada dia.

As novidades para hoje são, primiramente, que o Antonio está, PERNAMENTEMENTE, fora do SIPAP. Ele ainda está recebendo oxigênio através do CPAP, que mantém uma pressão continua no pulmão para evitar que ele colapse e também que o Antonio entre em apnéia. A máquina não força mais nenhuma respiração por minuto (o Antonio recebia 10 rpm até hoje pela manhã).

Essa mudança só ocorreu por volta das 9:00, então ainda é cedo pra ver como ele via reagir. A Enf. Virginia que está tomando conta dele hoje me disse que normalmente os bebês precisam receber mais O2 quando fazem essa mudança, mas nem isso o Antonio precisou.

A outra mudança foi em relação à alimentação: Os médicos começaram a consolidar a alimentação do Antonio, que agora prssará a receber o leite materno à razão de 11 mL/hora por três horas, e descansará por uma hora -- antes ele estava recebendo o leite durante 11 horas, com uma hora de descanso. O leite ainda está sendo dado através da sonda.

Mais uma vez, ainda é cedo pra julgar como ele vai receber essa "dieta", mas esse é o caminho pra ele começar a se alimentar normalmente. Então ele ficará nesse regime por alguns dias, e depois os médicos vão aumentando o volume e diminuindo o intervalo da alimentação, até ele começar a receber leite por uma hora e descansar por três.

The great news for today is that, first, Antonio is out of the SIPAP for good. He is still on the CPAP to keep his airway open under air pressure, and reducing the risk of the lungs collapse and of apneas. The machine still keep some O2 pressure, but is not giving him any more breathes per minute -- he was getting 10 bpm until this morning.

This change was made around 9am, so it is still early to tell how Antonio will react. Nurse Virginia told me that most of the time the babies need to get additional O2 when a change like this is made, but not even that was required for him... it's a good sign.

Another big change is related to his feedings: The doctors decided to start consolidating his feedings, and Antonio is now receiving milk at the rate of 11 mL/hour for three hours, then he will have a 1 hour break and start all over again.

Again, it is still early to tell how he will react to this new "diet," but that is the plan to get him to eat normally... so he will be getting this regimen for a few days and then they will increase the volume of milk and reduce the time of the feeding, until Antonio starts to eat for one hour and rest for three.
