So, since the sun was not so strong today but it was still hot today, we decided to let Antonio to take his first dip in the pool... at first he did not like it because of the temperature shock (the water ws at 82 degrees F), but then he kind of started enjoying himself.




Hoje aproveitamos que o sol não estava muito forte pra botar o Antonio na piscina pela primeira vez... a princípio ele não gostou muito pois levou um "choque de temperatura" (a água estava à 28 graus C) mas aos poucos se acostumou e começou a gostar um pouco mais.

All is good & Está Tudo bem



Acabamos de voltar do Hospital, e apesar do Antonio ainda estar um pouquinho "grog" desde ontem, ele já está aparentando estar muito melhor... até sorrindo ele já está!

A noite foi um pouco difícil, pois após passar o dia todo sem receber oxigênio pela cânula, os médicos reolveram testar e ver se ele conseguiria passar a noite também. Com isso, mais o fato da anertesia começar a passar, o Antonio estava com alguma dificuldade em manter a saturação do O2. Mas isso foi resolvido rapidamente or voltas das 22:00 quando a enfermeira da noite resolveu dar um "break" e colocar a cânula.

Depois disso, ele dormiu a maior parte da noite apenas acordando algumas vezes para comer. Às 4 da manhã, ele começou a reclamar de dores mais uma vez, mais nada que uma dose de Tylenol com Codeína -- combiado com colo do pai -- não resolvesse.



Ele finalmente acordou as 7:00, comeu muito bem e ficou alerta o tempo todo até chegarmos em casa às 9:30.

Eu não vou colocar nenhuma foto, mas a cirurgia correu muito bem: a melhor descrição é que antes da cirurgia o saquinho do Antonio parecia -- em forma e tamanho -- com dois kiwis lado-a-lado! Agora e apesar dele ainda estar inchadinho pro causa de fluidos, a "área" já está muito menor -- Nas palavras bem técnicas do cirurgião:

"O Saco dele já não é tão impressionante!"



We just arrived back from the Hospital, and although Antonio is still a bit "off" since yesterday, he is starting to look more like his "old self" ... even started smiling at us a little!



Last night was a long one because, after spending all day long without receiving any O2, the doctors and nurse decided to try and see if he would do well without it during the night. That, in addition to the anestesia wearing of and Antonio hurting a little from the surgery, made him have some difficulty in breathing and his o2 saturation started to fall below 92% and the nurse quickly decided to give him some help and put a fresh cannula on him around 10pm.

After this, he slept for most of the night waking up briefly a few times to eat a little. Up to four in the morning, when Antonio started to "complain" about pain, but nothing that a dose of Tylenol with Codeine -- combined with dad's hug and lap --did not cure.

He woke up the 7:00 am, ate very well and was alert all the time until arriving home around at the 9:30 am.



I will not upload a picture, but the surgery ran very well: the best description is that before the surgery Antonio, let's say, balls looked like -- both in form and size -- with two kiwi fruits side-by-side! Now and although the area is still swollen due to fluids, it does look much better -- The surgeon described it like this:

"They are not that impressive any more!"

Hoje é "Dia das Hérnias", então o blog estará em silêncio até que o Antonio saia da sala de cirurgia... aos amigos e "passantes", peço que mantenham aquela corrente pra frente a partir da 11:30 horário de Brasília. Obrigado!

Antonio! Listen to me Son... Escuta aqui meu filho!

Great, one less thing for us to worry about... after fussing for a while, Antonio agreed and took the hearing test -- twice for each ear, and PASSED! Not that we thought he could not hear, but it is required and it is good to know that his ears are functioning.























After that was finished, we went to see Antonio's "girlfriends" at St. Joseph's -- Valerie, Cathy, Dr. Bennett, Melissa, Barbara, Edna, Aileen... Antonio was very happy to see all of them, but in the end he was like "Where's Bridget? Why does she not call me? Does she have somebody else already?"

Boas notícias... após reclamar um bocado, o Antonio finalmente resolveu cooperar com a médico, e fez o teste de audição -- duas vezes para cada ouvido, e PASSOU!!! Não que nós achávamos que ele não conseguia ouvir, mas é bom saber que os ouvidos dele funcionam bem!

Depois que o exame dele acabou, demos um pulinho no Hospitad St. Joseph para uma visita às namoradas do Antonio: ele ficou contente em ver a Dra. Bennett, Valerie, Barbara, Cathy, Edna.. mas no final reclamou muito: "Onde está a Bridget? Por que ela nào me liga? Ela já tem outro?

42 ... Dias Depois & Days After



Hoje fazem 42 dias desde que ele veio pra casa e levamos o Antonio ao pediatra mais uma vez, para fazer um exame pré-operatório para a cirurgia da quarta-feira... tudo está indo muito bem, e ele cotinua ganhando peso e crescendo -- estava com 5.216 gramas e 52.7 cm de comprimento.



Amanhã o levaremos para fazer exame de audição e aproveitaremos para fazer uma visitinha rápida ao time do NICU em St. Joseph's, pois será lá o exame... depois disso, tentar passar o dia descansando pois será aquela agonia até a quinta-feira de manhã quando o Antonio (se Deus quiser) tiver alta do hospital após a cirurgia. Sem contar com a agonia do jogo em Quito...

It has been 42 days since that Antonio came home and to celebrate we took him again to see his pediatrician, to have a pre-op examination for Wednesday’s surgery … everything is going very well, and he continues gaining weight and growing – he was 11 lbs 8 oz and 20 ¾ inches long!

We will take him to have his hearing test tomorrow, and since that is close to St. Joseph’s Hospital, we will sneak in a quick visit to the NICU’s staff… after that, we will go home and try to relax before the surgery, but I will be in literally in agony until the Thursday morning when Antonio leaves the hospital again!

Tchau Vó & Ciao Grandma!



Dez dias passaram muito rápido... demais! Já estamos morrendo de saudades, e a casa ficou ainda mais quieta do que já era, pois a minha mãe "fazia uma festa" com o nosso pequeno Antonio: cantava, fazia vozes dele reclamando do pai, balançava ele no bebê conforto, nos braços... enfim ela fazia muito bem o trabalho de vó coruja!

Como na chegada dela, a levei até o aeroporto JFK em Nova York, e mesmo tendo que dirigir por mais de seis hora (ida-e-volta) eu gostaria de que a viagem até lá divesse durado umas horinhas mais, para que eu pudesse curtir minha mãezona por mais tempo!

The past ten days have gone fast… very fast! We already miss grandma Margarida, and the house is even quieter than before she was here, because she partied with Antonio all of the time, singing songs, translating Antonio's cries into "Daddy, where is my food, Daddy I am hungry!!!", she rocked him in her arms for comfort… and at last she did her grandmother job very well! Just as when she arrived I had to drive her to JFK airport in New York, and the same six hours round trip drive. I wish that the trip was longer so that I could have enjoyed my mom's company a little bit more!




Foi uma visita rápida, mas deu pra matar um pouco da saudades que eu sinto... esse é realmente o pior dos aspectos de se morar fora do Brasil: a falta da convivência com a família e os amigos. Ainda mais com minha mãe, por que eu sempre foi muito apegado com ela e sei que a minha vinda aos EEUU me roubou anos preciosos da vida dela.

It was a very quick visit, and not quite enough to make me miss her a little less. This is the worst part of living outside of Brazil:not being able to enjoy my family and friends especially my mother since I have always been very close to her, and I know that my coming to the United States stole precious time I could have spent with her.

Não falei que voltaria pra postar essa notícias? Então aqui vão elas, mas senta por que o post será longo...

Pra começar, o bichinho é bom de garrafa! desde que começamos a dar a fórmula Enfamil pra ele, ele tomou gosto pela comida e passou a mamar entre 100 e 125 ml! Mesmo nas madrugadas, quando era uma M acordá-lo e fazer com que ele mamasse mais que 30 ml, agora ele traça 60-90 ml sem nenhum problema!



Finalmente conseguimos marcar a cirurgia para as hérnias do Antonio... Será no dia 25/6, e segundo o cirurgião Dr. Buck o Antonio só precisará passar uma noite no Hospital. Ele também disse que não vê a possibilidade de nenhuma complicação, mas que considerando o tamanho das ditas cujas, existe o risco de elas reaparecerem mais tarde. Então vocês já sabem, no dia 25 às 11:30 da manhã horário de BSB, é aquela corrente pra frente...

Outra boa novidade é que tivemos nossa primeira visita de trabalho do programa "Bebês & Crianças." Vieram a fisioterapeuta e a professora para avaliar o Antonio e ambas acharam que ele está avançado em relação a sua idade ajustada de 1 mês!!! A professora acha que ele está bem desenvolvido tanto na parte social (equivalente à 4 meses de idade) e motora (2-3 meses); A fisioterapeuta disse que precisamos trabalhar alguns pontos, mas que no geral tanto a "postura" quanto o tônus dos músculos do Antonio estão muito bons... mais tarde no verão ela via botar a piscina pra funcionar" e nos ensinar a fazer hidroginástica com ele.

Agora a melhor parte: ontem o levamos ao pulmonologista pela primeira vez e a Dra. Sterni, que trabalha em John Hopkins, e ela ficou impressionada com o progresso do Antonio em relação a história clínica dele. Tão impressionada que fez uma porção de mudanças, para melhor:


  1. Parou com as nebulizações e descontinuou o uso do Xopinex, além de trocar o Pulmicort pelo inalador em aerosol Flovent HFA (Flixotide) o que facilitará muito a nossa vida, pois requer apenas duas bombadas duas vezes por dia, enquanto era duas brigas de 15 minutos com o para fazer cada nebulização!


  2. Começou a diminuir o uso de outro medicamentos, e passaremos a dar Potassio apenas 2 vezes por dia (era 4x antes), além não ajustar os diuréticos para o peso atual do Antonio (4.621 gramas). Ela também falou que vai descontinuar um dos diuréticos após a cirurgia das hérnias.


  3. A partir de ontem, podemos desconectar o monitor cardíaco sempre que o Antonio estiver acordado ou sendo "vigiado" de perto... nem preciso dizer o quanto essa mudança me faz contente, não só por eu estar sempre tropeçando e me enrolando no cabo do monitor, mas principalmente pelo significado para nós de que os pulmãos e o coração dele estão funcionando bem.


  4. Nós testamos a saturação de 02 no sangue do Antonio sem o auxílio do tubo de oxig6enio por mais de 45 minutos e durante a amamentação também. Como a saturação nunca caiu pra menos de 93%, a Dra. Sterni também pensou a descontinuar o uso do oxigênio durate o dia (só o colocariamos para ele ir dormir), mas por causa da cirurgia ela resolveu espera mais 2 ou 3 semanas antes de fazer essa mudança.


  5. Além disso, para a nossa próxima visita daqui a 2 meses, ela "encomendou" uma polisomnografia pra ver como a respiração e o nível de 02 dele se mantém durante o sono. Se tudo se sair como a Dra. Sterni acha que vai, o Antonio não deverá mais usar nem o monitor, nem o tubo de oxigênio até o final do verão!!!!


Bem, de resto nosso pequeno tricolor está aproveitando a visita da vovó pra ficar ainda mais manhoso ... Mas vó é pra isso mesmo, certo?



Did I not say that I would come back to post more news? So here goes, but you should sit down because this is a long post...

For an appetizer, the little guy can now hold his drink!!! Since we started giving him Enfamil formula, he got hooked on it and started to suck down between 3 1/2 and 4 Oz per feeding! Even over night, when it always has been a fight to get him to eat more than 1 Oz (even at the NICU!), now Antonio wolfs downs 2 and 3 Oz without any problem!

We were finally able to schedule his hernia surgery which will be on 6/25. According to Dr. Buck (surgeon), Antonio will only need to spend one night in the hospital, which for us will be a long time already! He also said that he does not see any issues or complications, but considering the size of those "puppies", there is the risk them reappearing later.

More good news is that we had our first visit from the Infant & Toddler program, with both a physical therapist and a teacher coming in to evaluate Antonio. Both said that in reality he is ahead of the game in relation to his adjusted age of 1 month! The teacher said that he has "social skills" of a four-month old, and some of his motor skills are appropiated to a two- or three-month old baby!!! The physical therapist said that we will need to work out some of his joints and muscles, but that overall both his posture and muscle tone are pretty good... later in the summer she intends to put the swimming pool to good use and we will do hydrogymnastics with him.

Now the best part: yesterday we took Antonio to the pulmonologist for the first time and Dr. Sterni, that works for Johns Hopkins, was very impressed with the progress of Antonio in relation his clinical history, so impressed that she made a bunch of changes, for the better:

1.She stopped the use of the nebulizer and it discontinued the use of the Xopinex and instead of giving him Pulmicort we will be giving him an inhaler called Flovent. That will make all of our lives much easier. Because before we had to fight with him to administer the 15 minute treatment while he was swaddled and blindfolded, now we only have to use an aerochamber and administer 2 puffs and make sure he takes 10 breaths while he has the mask covering his nose and mouth.

2. She also started to diminish the use of his other medicines, and we will start to give his Potassium only 2 times per day (he was 4x before), and going forward she will not adjust the diuretics for his current weight of 10 lb 3 oz. but she intends to let him grow out of them!

3. She also mentioned that she might discontinue the use of one of the diuretics after his hernia surgery.

4. As of yesterday, Antonio only has to be on the Apnea monitor when he is sleeping or when he is unattended. This is great news because it will make our lives much easier because we won't be stumbling over the cords all of the time and getting twisted with his oxygen tubing, but mainly because it means that his lungs and heart are functioning well.

5.We also tested the O2 saturation in his blood while he was there without being connected to oxygen for 45 minutes and while he ate. His saturation never fell less than 93%, Dr. Sterni also thought she might discontinue the use of the oxygen during the day (and only continue using it while he sleeps), but because of the hernia surgery scheduled in 2 weeks she decided to wait until we passed that hurdle before making this change.

6. Finally, our next visit will be in 2 months, and the doctor has already ordered a sleep study for Antonio. Basically this involves him coming in overnight to be watched while he sleeps without oxygen prior to discontinuing it completely. She told us that if he continues to do as well as he is she sees no reason why he can't be completely off of oxygen and the monitor by the end of the summer!

Well to finish our little guy is happy to spend time with his grandmother whom is spoiling him to death...but that is what grandmothers were made for right?

Nem vou me desculpar de novo pela demora, mas estou realmente sem muito tempo para atualizar o blog, apesar das muitas novidades desde que minha mãe chegou... Com certeza é coincidência, mas desde que ela começou a interagir com ele, o Antonio passou a ficar muito mais sociavel -- o que significa que ele sorrí mais para nós e até "gargalhadas" quando brincamos com ele!

Até agora a visita dela tem sido tranqüila, pois tivemos uns dias muito quentes, e portanto passamos muito tempo em casa... mas essa semana também foi um corre-corre danado com muitas visita aos especialistas -- o que será tratado em outro post.

De qualquer forma o Antonio continua progredindo e adorando a visita da vovó!





I will not try to excuse myself, but the fact is that I am really without much time to update the blog -- although we DO HAVE a lot news since my mother arrived... I am sure it is just a coincidence, but since she started interacting with him, Antonio became even more sociable -- which means that he smiles more often and even started to giggle when we play with him!

So far her visit has been calm, especially because the heat wave the last days precluded us from going out much... but this week also was very busy with several visits to the specialists -- which will be addressed in a separate post

In any case, Antonio continues to show progress and he loves spending time with grandma!

The Silence of the Baby & O Silêncio do Inocente Bebê

Hello Everyone!

I know we've been quite quiet, but between Antonio being at home (he's very demanding), a new client (even more demanding!) I got, and getting the house & yard ready for the summer, it has been difficult to keep up with the blog.

Anyway, so far our little guy is doing great, growing and gaining weight better then planned -- he was up to 9 lbs 7 oz last Saturday, really impressing nurse Bonnie, to the point of her mentioning that he probably will not get many more visits from her...

We went back to see Dr. Mike II for the third weekly visit, and he said Antonio continues to improve. There's no changes on his medication so far, but we made a small change on Antonio's diet: Because Sandra's milk production is slowly diminishing, we decide to introduce formula and are giving him that twice a day.

So, four times a day, he is getting anywhere between 2.5 and 4 Oz of breast milk enriched with rice cereal and Enfamil Human Milk Fortifier (to add 4 extra calories per ounce of milk), and two times a day he gets 1 oz of milk with his medicines and 3-4 oz of Enfamil EnfaCare 22 cal -- Enfamil is definitely the best formula in the market, and MeadJohnson is the best company in the world ;o))

The plug is because the rep for Mead Johnson has donated a few boxes of the Human Milk Fortifier to Antonio, which has been a great help -- each dose costs $1.10 and Antonio goes through 12-14 doses each day ... So, if you are reading this: THANK YOU, THANK YOU, THANK YOU!!!!!

Tomorrow, we will see Dr. Mike II again, as well as getting a visit from the coordinator of the Infant and Toddler Program for Harford County's Schools, who is coming to evaluate Antonio and propose some of the programs that will help on his development.

And last, but not least, my mom is arriving for a quick visit on Saturday -- she will stay with us for 10 days. It has been 10 years since her last visit to the USA, and last time things did not go so well... So I hope she will have a pleasant trip and stay with us. Antonio, Sandra, Sassy, Phoebe and I are very excited and looking forward to the visit and the help she will give us during her stay!

Olá a todos! Eu sei estamos estado muito quietos ultimamente, mas entre a chegada do Antonio em casa (ele é muito exigente), um cliente novo (que é mais exigente ainda!), e a necessidade de deixar a casa e o jardim prontos para o verão fica difícil patualizar o blog.

Em todo o caso, até agora nosso pequeno Antonio continua a crescer e ficar mais gordinho -- estava com 4.281 gramas no último sábado, impressionando a enfermeira Bonnie, ao ponto dela mencionar que provavelmente não precisará fazer muito mais visitas.

Tivemos mais uma visita ao Dr. Mike II que disse que Antonio continua indo bem... Não fizeram nenhums mudança nos medicamentos que ele toma, mas nós fizemos uma pequena alteração na dieta do Antonio: Uma vez que o suprimento de leite da Sandra vem dimunindo drasticamente, decidimos passar a dar fórmula a ele duas vezes por dia.

Dessa forma, quatro vezes por dia, o Antonio toma entre 80 e 120 ml de leite materno enriquecido com Cereal de Arroz e Fortificante de Leite Humano Enfamil (para adcionar 4 calorias por 30 ml de leite), e duas vezes ao dia ele toma 30 ml de leite com seus remédios mais 90-120 ml Enfamil EnfaCare 22 cal -- Enfamil é realmente a melhor fórmula no mercado, e MeadJohnson é a melhor companhia no mundo; o))

A propaganda acima é porque o representante para da MeadJohnson doou algumas caixas do Fortificante de Leite Humano ao Antonio, que foi uma grande ajuda -- cada pacotinho desse troço custa US$1.10 e o Antonio traça entre 12 e 14 pacotinhos por dia ... assim, se você está lendo este blog e fala protuguês: OBRIGADO, OBRIGADO, OBRIGADO!!!!!

Amanhã, nós veremos o Dr. Mike II outra vez, assim como receberemos a visitada coordenadora do programa "Infant & Toddler" da nossa área, que está vindo avaliar Antonio e propor alguns dos programas (gratuítos) que ajudarão em seu desenvolvimento.

E por último, minha mãe está chegando para uma visita rápida nesse sábado e ficará aqui por 10 dias. Já fazem 10 anos desde da sua última visita aos EUA, e as coisas da última vez não foram tão bem... Assim eu espero que desta vez seja diferente e que ela prossa curtir o netinho, ale'm a nora e do filho é claro! O Antonio, Sandra, Sassy, Phoebe e eu estamos contando os minutos para a chegada dela!
