WHAT'S COMING NEXT

Oh well, we knew it was coming to this... We just hung up the phone with phone with Dr. Bennett, and they will try to help Antonio have his best chance when they extubate him... here's their plans for the next 48-72 hours:

1. They checked his hematocrit this morning, and it was at 35% which is not too low for a baby his age, but normally it should be on the 40's. So they will give him another blood transfusion today to increase the number of red blood cells, which is the principal means of delivering oxygen from the lungs to the rest of the body.
   
   So, he is off the feedings for the day, as he cannot "eat" while getting the blood transfusion, but the good news is that he gained more weight since yesterday and he is up to 2 Lb 12 Oz.



2. They will start a short course of a steroid (decadron) for 2-3 days at the lower dose. This is a different treatement, with a much lower dosage and course, than what we discussed last week, but in any case, but the doctors feel that it will help Antonio’s lungs to work more effectively when he goes on the CPAP, since his issue is that his airway is inflammed and the swelling makes difficult for him to breathe.
   
   As for side effects, they do not think anything serious might happen because of the short course, but they will keep an eye for higher blood pressure, sugar levels, and stomach bleeds (the Zantac he is taking will help with that as well). Also, this medication can increase his succeptibility for infections.

That's it... 24-28 hours after they started these treatments, if everything goes as planned and he stays the course, they will remove the ET tube. That is makes for a interesting Saturday morning, as it should happen then around 10 am. Of course, there may be the need to reintubate him, but we do not want to think or talk about this right now.

Bem, nós sabíamos que estava por vir.... Acabamos de desligar o telefone com a Dra. Bennett, que nos informou que a partir de hoje estarão começando uma nova guerra pra dar a melhor chance do Antonio graduar do Ventilador... esses são os planos para as próximas 48-72 horas:

1. Um examde de sangue hoje pela manhã mostrou que o nível de hematócritos esta manhã estava em 35%, que não é tão baixo para um bebê da idade do Antonio, mas normalmente deveria estar em 40%. Com isso resolveram dar-lhe mais uma transfusão de sangue hoje para aumentar o número de glóbulos vermelhos que, como sabemos, transporta o oxigênio dos pulmões para os tecidos.
   
   Uma vez que ele está recebendo sangue, tiveram que parar com a alimentação, mas a boa notícia é que ele ganhou mais peso desde ontem e está agora pesando 1.250 gramas.
   
   
2. Começarãoa dar ao Antonio um curso curto de um esteroide (decadron) por 2-3 dias numa dosagem baixa. Este é um tratamento diferente, com dosage muito mais baixa e curso mais rápido, do que o que nós discutimos a última semana, mas em todo o caso, os médicos acreditam que ajudará os pulmões do Antonio trabalhar mais eficazmente quando ele começar a usar o CPAP. Isso por que eles acham que o problem do Antonio não é mais o pulmão, mas as vias respiratórias que estão imflamadas e inchadas, dificultado a respiração.
   
   Quanto aos possíveis efeitos colaterais, os médicos não acreditam que possam ocorrer qualquer coisa mais séria, mas vão manter o olho na uma pressão sanguínea, níveis de açúcar, e para sangramento no estômago (o Zantac que ele está tomando agora vai ajudar nisso). Além disso, este medicamento pode aumentar a possibilidade do antonio contrair uma infecção.

That's it... após 24-28 horas do começo desse tratamento, se tudo correr como planejado e ele continue nesse curso, elas removerão o tubo endotraqueal do Antonio.

Vai ser um sábado daqueles, pois isso deve por volta das 10:00. Naturalmente, pode ser que tenham que reintubá-lo, mas nós nem queremos pensar ou falar sobre isso agora.
