Chamando Dr. House / Paging Dr. House

Não falei que voltaria pra postar essa notícias? Então aqui vão elas, mas senta por que o post será longo...

Pra começar, o bichinho é bom de garrafa! desde que começamos a dar a fórmula Enfamil pra ele, ele tomou gosto pela comida e passou a mamar entre 100 e 125 ml! Mesmo nas madrugadas, quando era uma M acordá-lo e fazer com que ele mamasse mais que 30 ml, agora ele traça 60-90 ml sem nenhum problema!



Finalmente conseguimos marcar a cirurgia para as hérnias do Antonio... Será no dia 25/6, e segundo o cirurgião Dr. Buck o Antonio só precisará passar uma noite no Hospital. Ele também disse que não vê a possibilidade de nenhuma complicação, mas que considerando o tamanho das ditas cujas, existe o risco de elas reaparecerem mais tarde. Então vocês já sabem, no dia 25 às 11:30 da manhã horário de BSB, é aquela corrente pra frente...

Outra boa novidade é que tivemos nossa primeira visita de trabalho do programa "Bebês & Crianças." Vieram a fisioterapeuta e a professora para avaliar o Antonio e ambas acharam que ele está avançado em relação a sua idade ajustada de 1 mês!!! A professora acha que ele está bem desenvolvido tanto na parte social (equivalente à 4 meses de idade) e motora (2-3 meses); A fisioterapeuta disse que precisamos trabalhar alguns pontos, mas que no geral tanto a "postura" quanto o tônus dos músculos do Antonio estão muito bons... mais tarde no verão ela via botar a piscina pra funcionar" e nos ensinar a fazer hidroginástica com ele.

Agora a melhor parte: ontem o levamos ao pulmonologista pela primeira vez e a Dra. Sterni, que trabalha em John Hopkins, e ela ficou impressionada com o progresso do Antonio em relação a história clínica dele. Tão impressionada que fez uma porção de mudanças, para melhor:

1. Parou com as nebulizações e descontinuou o uso do Xopinex, além de trocar o Pulmicort pelo inalador em aerosol Flovent HFA (Flixotide) o que facilitará muito a nossa vida, pois requer apenas duas bombadas duas vezes por dia, enquanto era duas brigas de 15 minutos com o para fazer cada nebulização!



2. Começou a diminuir o uso de outro medicamentos, e passaremos a dar Potassio apenas 2 vezes por dia (era 4x antes), além não ajustar os diuréticos para o peso atual do Antonio (4.621 gramas). Ela também falou que vai descontinuar um dos diuréticos após a cirurgia das hérnias.



3. A partir de ontem, podemos desconectar o monitor cardíaco sempre que o Antonio estiver acordado ou sendo "vigiado" de perto... nem preciso dizer o quanto essa mudança me faz contente, não só por eu estar sempre tropeçando e me enrolando no cabo do monitor, mas principalmente pelo significado para nós de que os pulmãos e o coração dele estão funcionando bem.



4. Nós testamos a saturação de 02 no sangue do Antonio sem o auxílio do tubo de oxig6enio por mais de 45 minutos e durante a amamentação também. Como a saturação nunca caiu pra menos de 93%, a Dra. Sterni também pensou a descontinuar o uso do oxigênio durate o dia (só o colocariamos para ele ir dormir), mas por causa da cirurgia ela resolveu espera mais 2 ou 3 semanas antes de fazer essa mudança.



5. Além disso, para a nossa próxima visita daqui a 2 meses, ela "encomendou" uma polisomnografia pra ver como a respiração e o nível de 02 dele se mantém durante o sono. Se tudo se sair como a Dra. Sterni acha que vai, o Antonio não deverá mais usar nem o monitor, nem o tubo de oxigênio até o final do verão!!!!

Bem, de resto nosso pequeno tricolor está aproveitando a visita da vovó pra ficar ainda mais manhoso ... Mas vó é pra isso mesmo, certo?



Did I not say that I would come back to post more news? So here goes, but you should sit down because this is a long post...

For an appetizer, the little guy can now hold his drink!!! Since we started giving him Enfamil formula, he got hooked on it and started to suck down between 3 1/2 and 4 Oz per feeding! Even over night, when it always has been a fight to get him to eat more than 1 Oz (even at the NICU!), now Antonio wolfs downs 2 and 3 Oz without any problem!

We were finally able to schedule his hernia surgery which will be on 6/25. According to Dr. Buck (surgeon), Antonio will only need to spend one night in the hospital, which for us will be a long time already! He also said that he does not see any issues or complications, but considering the size of those "puppies", there is the risk them reappearing later.

More good news is that we had our first visit from the Infant & Toddler program, with both a physical therapist and a teacher coming in to evaluate Antonio. Both said that in reality he is ahead of the game in relation to his adjusted age of 1 month! The teacher said that he has "social skills" of a four-month old, and some of his motor skills are appropiated to a two- or three-month old baby!!! The physical therapist said that we will need to work out some of his joints and muscles, but that overall both his posture and muscle tone are pretty good... later in the summer she intends to put the swimming pool to good use and we will do hydrogymnastics with him.

Now the best part: yesterday we took Antonio to the pulmonologist for the first time and Dr. Sterni, that works for Johns Hopkins, was very impressed with the progress of Antonio in relation his clinical history, so impressed that she made a bunch of changes, for the better:

1.She stopped the use of the nebulizer and it discontinued the use of the Xopinex and instead of giving him Pulmicort we will be giving him an inhaler called Flovent. That will make all of our lives much easier. Because before we had to fight with him to administer the 15 minute treatment while he was swaddled and blindfolded, now we only have to use an aerochamber and administer 2 puffs and make sure he takes 10 breaths while he has the mask covering his nose and mouth.

2. She also started to diminish the use of his other medicines, and we will start to give his Potassium only 2 times per day (he was 4x before), and going forward she will not adjust the diuretics for his current weight of 10 lb 3 oz. but she intends to let him grow out of them!

3. She also mentioned that she might discontinue the use of one of the diuretics after his hernia surgery.

4. As of yesterday, Antonio only has to be on the Apnea monitor when he is sleeping or when he is unattended. This is great news because it will make our lives much easier because we won't be stumbling over the cords all of the time and getting twisted with his oxygen tubing, but mainly because it means that his lungs and heart are functioning well.

5.We also tested the O2 saturation in his blood while he was there without being connected to oxygen for 45 minutes and while he ate. His saturation never fell less than 93%, Dr. Sterni also thought she might discontinue the use of the oxygen during the day (and only continue using it while he sleeps), but because of the hernia surgery scheduled in 2 weeks she decided to wait until we passed that hurdle before making this change.

6. Finally, our next visit will be in 2 months, and the doctor has already ordered a sleep study for Antonio. Basically this involves him coming in overnight to be watched while he sleeps without oxygen prior to discontinuing it completely. She told us that if he continues to do as well as he is she sees no reason why he can't be completely off of oxygen and the monitor by the end of the summer!

Well to finish our little guy is happy to spend time with his grandmother whom is spoiling him to death...but that is what grandmothers were made for right?
